Turning to the statistical, heuristic, exercise carried out on the French data, the surprising emergence of three such distinct groups of women leads to the speculation that it is perhaps not one premenstrual syndrome with which we are dealing - but three. It would not be unthinkable that each of these three syndromes has its own aetiology, and, therefore, that each requires a different therapy. This might explain the variety in the ways in which women expenence the premenstrual syn40 AN EPIDEMIOLOGICAL AND STATISTICAL EXERCISE drome.

An analysis of the women who suffered skin problems showed that this was primarily a complaint of women over the age of 48; one wonders if this is not, in fact, a climacteric symptom rather than a premenstrual one. This is purely a matter of conjecture, however. 37 THE PRE'VIE"STRUAL SY:-\OROME The next question was: Are there any explanations in the data available which might 'explain' why a woman is in one group as opposed to another, or rather, given the nature of the data available, are there any factors which dominate in any of the groups which might indicate the likelihood of a woman being in that group as opposed to any of the others?

The severity of the syndrome will be the product of the degree of changes in the internal environment and the reaction of the patient as determined by her personality, stability and emotional state. ' Any rationale for progesterone therapy for premenstrual syndrome must be to alter what Rees describes as the 'internal environment'. e. progesterone, although it has some sedative properties, is not an anxiolytic, analgesic or antidepressant. The double-blind controlled trials suggest that progesterone given in the luteal phase is as effective as placebo, and this would suggest it is affecting the 'reaction of the patient' and not the 'internal environment'.
